x86/kvm: Don't consume async #PF flags on a kernel #PF in NMI context

An async #PF "page not present" event arrives through the #PF vector,
and its reason sits in the per-CPU apf_reason.flags until
__kvm_handle_async_pf() reads and clears it. If an NMI arrives after
the event is injected and before the #PF handler reads the flags, and
the NMI handler itself takes a #PF, that fault reads and clears the
flags belonging to the #PF it interrupted. It then finds interrupts
disabled in the NMI context it interrupted, and panics:

 Kernel panic - not syncing: Host injected async #PF in interrupt
disabled region
 Call Trace:
 <NMI>
 panic+0x52/0x60

 exc_nmi+0xe8/0x110
 RIP: 0010:asm_exc_page_fault+0x0/0x30
 </NMI>

The last RIP line shows the NMI landing on the first instruction of
the #PF entry. perf with user call chains takes such a fault: the
sampling NMI walks the user stack with __get_user(), which faults when
a saved frame pointer is not a valid user address or the walk runs off
the top of the stack. The same window exists when KVM, running as an
L1 hypervisor with DELIVERY_AS_PF_VMEXIT, reads the flags itself after
a #PF VM-exit.

The NMI entry code saves and restores CR2 for this nesting, but not the
async #PF reason. Andy Lutomirski described the race, with perf as the
example, when async #PF delivery was reworked in 2020 [1]. Saving and
clearing the flags across the NMI, next to CR2, was discussed there but
not merged.

Since that rework, Linux has stopped setting KVM_ASYNC_PF_SEND_ALWAYS,
so the host delivers "page not present" only for user mode faults and
a kernel-mode fault in NMI context (including kernel-mode #MC and #DB)
cannot be an async page fault. In that case, return without touching
the flags so that the interrupted handler finds them intact.

Tested on v7.3-rc5 in a 4-vCPU, 2 GiB guest restored from a snapshot
whose memory is served on demand through userfaultfd, so that reading
it back takes async #PFs, while every thread samples hardware cycles
with user call chains and points its frame pointer at an unmapped page.
Without this patch the guest panicked as above in 3 of 3 runs. With it,
the guest survived 3 of 3 runs of 60 seconds, and the host counted
148,441 to 153,334 kvm_async_pf_not_present events in the first 20
seconds of each. On 6.18.50 the results were the same: the guest
panicked without the patch and survived with it.

The "cs" prefix on the LEA in the FineIBT paranoid caller template is
padding: it places the LEA's 0xf0 immediate byte where it doubles as the
LOCK prefix that makes the failing Jcc raise #UD. GNU as warns about it
in every CONFIG_FINEIBT=y build:

 Warning: segment override on `lea' is ineffectual

Clang's integrated assembler does not warn, and both assemble the same
bytes. Emit the prefix with ".byte 0x2e" to silence GNU as.

Build tested ARCH=x86_64 defconfig with CONFIG_CFI=y, with GCC 17.0.0
(experimental, with the kCFI series) and GNU as 2.47, and with Clang
24.0.0git: GNU as no longer warns, and the template assembles to the
same bytes as before with both. Under QEMU with cfi=fineibt,paranoid,
the paranoid caller check catches LKDTM's CFI_FORWARD_PROTO with both.

Since commit b2250d63e6422 ("x86/kcfi: Optimize call sequence"), a
CONFIG_FINEIBT=y kernel running in kCFI mode rewrites each kCFI call
site at boot so that a failed check traps on the 0xd6 byte of
"test $0xd6, %al", one byte past where the compiler's "ud2" was.
handle_cfi_failure() steps "addr" back one byte to find the trap in
.kcfi_traps, but decode_cfi_insn() still decodes from regs->ip, so it
reads each instruction starting at its second byte.
"movl $-<id>, %r10d" then decodes as "movl $-<id>, %edx", so the
reported type is right by accident, but "addl -15(%r11), %r10d" decodes
as "addl -15(%rbx), %edx", so the reported target is whatever %rbx holds:

 CFI failure at lkdtm_indirect_call+0x16/0x20 (target: 0x12; expected type: 0xc79a2d1b)

Pass the adjusted address to decode_cfi_insn() and decode from it.

Tested ARCH=x86_64 defconfig with CONFIG_CFI=y and CONFIG_LKDTM=y under
QEMU, with LKDTM's CFI_FORWARD_PROTO and both Clang 24.0.0git and GCC
17.0.0 (experimental, with the kCFI series). With CONFIG_FINEIBT=y in
kCFI mode, the report gives %rbx as the target without this change, and
the expected lkdtm_increment_int() with it. With cfi=fineibt, with
cfi=fineibt,paranoid, and with CONFIG_FINEIBT=n, it gives
lkdtm_increment_int() both with and without this change.

get_align_mask() returns huge_page_mask_align() for hugetlbfs, but get_align_bits()
adds va_align.bits regardless, so vm_unmapped_area() returns an address off the huge
page boundary and __unmap_hugepage_range() hits BUG_ON(start & ~huge_page_mask(h)) at
teardown.

This can be triggered on Carrizo and FX-8370E, both hstates.

Pass the file to get_align_bits() and skip the randomisation for hugetlbfs.

x86/mce: Fix hardware debug register corruption on task migration

In exc_machine_check_user(), local_db_save() and local_db_restore() are
invoked in the outer entry stubs (DEFINE_IDTENTRY_MCE_USER,
DEFINE_FREDENTRY_MCE, and DEFINE_IDTENTRY_RAW), surrounding
exc_machine_check_user().

However, exc_machine_check_user() calls irqentry_exit_to_user_mode(), which
handles pending thread work and may schedule() if TIF_NEED_RESCHED is set. If
the task migrates to another CPU during schedule(), local_db_restore() runs on
the new CPU with the dr7 state saved from the old CPU. This corrupts the new
CPU's DR7 hardware debug register and leaves the old CPU's DR7 disabled. In
short, local_db_save() and local_db_restore() pair must be run on the same
CPU.

To fix this, move local_db_save() and local_db_restore() inside
exc_machine_check_user() and exc_machine_check_kernel(). In
exc_machine_check_user(), DR7 is saved and restored strictly around
do_machine_check() to avoid schedule() during migration. In
exc_machine_check_kernel(), local_db_save() is called at the entry point to
prevent early memory accesses from triggering nested #DB exceptions, and
restored on all exits.
