We need to use this function in common code, so define it for
architectures and/or configrations that miss it.  The result of
pmd_pfn() will only be used if TRANSPARENT_HUGEPAGE is enabled,
but a function or macro called pmd_pfn() must be defined, even
on machines with two level page tables.

Currently most platforms define FIRST_USER_ADDRESS as 0UL duplication the
same code all over.  Instead just define a generic default value (i.e 0UL)
for FIRST_USER_ADDRESS and let the platforms override when required.  This
makes it much cleaner with reduced code.

The default FIRST_USER_ADDRESS here would be skipped in &lt;linux/pgtable.h&gt;
when the given platform overrides its value via &lt;asm/pgtable.h&gt;.

Patch series "mm: clean up names and parameters of memmap_init_xxxx functions", v5.

This patchset corrects inappropriate function names of memmap_init_xxx,
and simplify parameters of functions in the code flow.  And also fix a
prototype warning reported by lkp.

This patch (of 5);

Kernel test robot calling make with 'W=1' is triggering warning like
below for memmap_init_zone() function.

  mm/page_alloc.c:6259:23: warning: no previous prototype for 'memmap_init_zone' [-Wmissing-prototypes]

Fix it by adding the function declaration in include/linux/mm.h.  Since
memmap_init_zone() has a generic version with '__weak', the declaratoin in
ia64 header file can be simply removed.

<title>arch/ia64: Restore arch-specific pgd_offset_k implementation</title>

IA-64 is special and treats pgd_offset_k() differently to pgd_offset(),
using different formulae to calculate the indices into the kernel and user
PGDs.  The index into the user PGDs takes into account the region number,
but the index into the kernel (init_mm) PGD always assumes a predefined
kernel region number. Commit 974b9b2c68f3 ("mm: consolidate pte_index() and
pte_offset_*() definitions") made IA-64 use a generic pgd_offset_k() which
incorrectly used pgd_index() for kernel page tables.  As a result, the
index into the kernel PGD was going out of bounds and the kernel hung
during early boot.

Allow overrides of pgd_offset_k() and override it on IA-64 with the old
implementation that will correctly index the kernel PGD.

All architectures define pte_index() as

	(address &gt;&gt; PAGE_SHIFT) &amp; (PTRS_PER_PTE - 1)

and all architectures define pte_offset_kernel() as an entry in the array
of PTEs indexed by the pte_index().

For the most architectures the pte_offset_kernel() implementation relies
on the availability of pmd_page_vaddr() that converts a PMD entry value to
the virtual address of the page containing PTEs array.

Let's move x86 definitions of the PTE accessors to the generic place in
&lt;linux/pgtable.h&gt; and then simply drop the respective definitions from the
other architectures.

The architectures that didn't provide pmd_page_vaddr() are updated to have
that defined.

The generic implementation of pte_offset_kernel() can be overridden by an
architecture and alpha makes use of this because it has special ordering
requirements for its version of pte_offset_kernel().
