Introduce macros that operate on a (start, end) range of GPRs, which
reduces lines of code and need to do mental arithmetic while reading the
code.

This patch moves ASM_CONST() and stringify_in_c() into
dedicated asm-const.h, then cleans all related inclusions.

This is the assembler code for the MD5 implementation.
Handling of algorithm constants has been slightly
changed to reduce register usage and make better use
of cores with multiple ALUs. Thus they are stored as
delta values.
