Commit f959dcd6ddfd29235030e8026471ac1b022ad2b0 (dma-direct: Fix
potential NULL pointer dereference) added a null check on the
dma_mask pointer of the kernel's device structure.

Add a dma_mask variable to the ps3_dma_region structure and set
the device structure's dma_mask pointer to point to this new variable.

Fixes runtime errors like these:

To aid debugging, add a new PS3 kernel config option
PS3_VERBOSE_RESULT that, when enabled, will print more
verbose messages for the result of LV1 hypercalls.

<title>powerpc/ps3: make system bus's remove and shutdown callbacks return void</title>

The driver core ignores the return value of struct device_driver::remove
because there is only little that can be done. For the shutdown callback
it's ps3_system_bus_shutdown() which ignores the return value.

To simplify the quest to make struct device_driver::remove return void,
let struct ps3_system_bus_driver::remove return void, too. All users
already unconditionally return 0, this commit makes it obvious that
returning an error code is a bad idea and ensures future users behave
accordingly.

None of these files are actually using any __init type directives
and hence don't need to include &lt;linux/init.h&gt;.  Most are just a
left over from __devinit and __cpuinit removal, or simply due to
code getting copied from one driver to the next.

The one instance where we add an include for init.h covers off
a case where that file was implicitly getting it from another
header which itself didn't need it.

Currently the FLASH database is updated by the kernel using file operations,
meant for userspace only. While this works for us because copy_{from,to}_user()
on powerpc can handle kernel pointers, this is unportable and a bad example.
Replace the file operations by callbacks, registered by the ps3flash driver.
