Don't inherit headers "by chances" from asm/prom.h, asm/mpc52xx.h, asm/pci.h etc... Include the needed headers, and remove asm/prom.h when it was needed exclusively for pulling necessary headers. powerpc: retire sbc8641d board support

The support was for this was added to mainline over 12 years ago, in v2.6.26 [4e8aae89a35d] just around the ppc --> powerpc migration. I believe the board was introduced shortly after the sbc8548 board, making it roughly a 14 year old platform - with the CPU speed and memory size typical for that era. I haven't had one of these boards for several years, and availability was discontinued several years before that. Given that, there is no point in adding a burden to testing coverage that builds all possible defconfigs, so it makes sense to remove it. Of course it will remain in the git history forever, for anyone who happens to find a functional board and wants to tinker with it. powerpc: Don't use 'struct ppc_inst' to reference instruction location

'struct ppc_inst' is an internal representation of an instruction, but in-memory instructions are and will remain a table of 'u32' forever. Replace all 'struct ppc_inst *' used for locating an instruction in memory by 'u32 *'. This removes a lot of undue casts to 'struct ppc_inst *'. It also helps locating ab-use of 'struct ppc_inst' dereference. powerpc: Use a datatype for instructions

Currently unsigned ints are used to represent instructions on powerpc. This has worked well as instructions have always been 4 byte words. However, ISA v3.1 introduces some changes to instructions that mean this scheme will no longer work as well. This change is Prefixed Instructions. A prefixed instruction is made up of a word prefix followed by a word suffix to make an 8 byte double word instruction. No matter the endianness of the system the prefix always comes first. Prefixed instructions are only planned for powerpc64. Introduce a ppc_inst type to represent both prefixed and word instructions on powerpc64 while keeping it possible to exclusively have word instructions on powerpc32. powerpc: Use a macro for creating instructions from u32s

In preparation for instructions having a more complex data type start using a macro, ppc_inst(), for making an instruction out of a u32. A macro is used so that instructions can be used as initializer elements. Currently this does nothing, but it will allow for creating a data type that can represent prefixed instructions.
