<title>libertas: Firmware loading simplifications</title>

Remove the ability to pass module parameters with firmware filenames
for USB and SDIO interfaces.

Remove the ability to pass custom "user" filenames to lbs_get_firmware().

Remove the ability to reprogram internal device memory with a different
firmware from the USB driver (we don't know of any users), and simplify
the OLPC firmware loading quirk to simply placing the OLPC firmware
at the top of the list (we don't know of any users other than OLPC).

Move lbs_get_firmware() into its own file.

These simplifications should have no real-life effect but make the
upcoming transition to asynchronous firmware loading considerably less
painful.

mesh.c will be the file where we concentrate all mesh-related
code. This allows us to either add a KConfig entry for mesh and
makes matters easier for the cfg80211 transition.

Most of the 11d code was protected with an "if (priv-&gt;enable11d)" clause.
But there was no code that anywhere that was able to set this
variable to true. So all 11d code was dead for almost a year and no one
complained. That's enought incentive to remove this code.

Besides removing old cruft, we gain back the 11d capability in a common way
when we merge the cfg80211 functionality.

Add initial support for libertas devices using a GSPI interface. This has
been tested with the 8686.

GSPI is intended to be used on embedded systems. Board-specific parameters are
required (see libertas_spi.h).

Thanks to everyone who took a look at the earlier versions of the patch.
