Add a kernel-wide "phys_addr_t" which is guaranteed to be able to hold
any physical address.  By default it equals the word size of the
architecture, but a 32-bit architecture can set ARCH_PHYS_ADDR_T_64BIT
if it needs a 64-bit phys_addr_t.

This patch is the result of an automatic script that consolidates the
format of all the headers in include/asm-x86/.

The format:

1. No leading underscore. Names with leading underscores are reserved.
2. Pathname components are separated by two underscores. So we can
   distinguish between mm_types.h and mm/types.h.
3. Everything except letters and numbers are turned into single
   underscores.

Set __PAGE_OFFSET to the most negative possible address +
16*PGDIR_SIZE.  The gap is to allow a space for a hypervisor to fit.
The gap is more or less arbitrary, but it's what Xen needs.

When booting native, kernel/head_64.S has a set of compile-time
generated pagetables used at boot time.  This patch removes their
absolutely hard-coded layout, and makes it parameterised on
__PAGE_OFFSET (and __START_KERNEL_map).
