This patch converts the card object management with devres as a clean
up. The remove callback gets reduced by that.

This should give no user-visible functional changes.

Note that this converts only the mpu401 card driver. The mpu401_uart
component is still managed with snd_device. It's for the case where
the mpu401_uart component may be removed dynamically without the
actual device unbind.

MPU401 driver code contains a few assignments in if condition, which
is a bad coding style that may confuse readers and occasionally lead
to bugs.

This patch is merely for coding-style fixes, no functional changes.

For a sake of code simplification, remove the init and the exit
entries that do nothing.

Notes for readers: actually it's OK to remove *both* init and exit,
but not OK to remove the exit entry. By removing only the exit while
keeping init, the module becomes permanently loaded; i.e. you cannot
unload it any longer!

In preparation for unconditionally passing the struct timer_list pointer to
all timer callbacks, switch to using the new timer_setup() and from_timer()
to pass the timer pointer explicitly.

pnp_device_id are not supposed to change at runtime. All functions
working with pnp_device_id provided by <linux/pnp.h> work with
const pnp_device_id. So mark the non-const structs as const.
