Use new return type vm_fault_t for fault handler. For
now, this is just documenting that the function returns
a VM_FAULT value rather than an errno. Once all instances
are converted, vm_fault_t will become a distinct type.
Ref- commit 1c8f422059ae ("mm: change return type to vm_fault_t")
Previously vm_insert_page() returns err which driver
mapped into VM_FAULT_* type. The new function
vmf_insert_page() will replace this inefficiency by
returning VM_FAULT_* type.
vmf_error() is the newly introduce inline function
in 4.17-rc6.

This replaces the repetitive GPL-2.0 license text in code and header files
with the SPDX tags. Generated hardware headers aren't changed, as any changes
there need to be done in the upstream rnndb repository.

I'm not aware of any case where tracing GPU register manipulation at the
kernel level would have been useful. It only adds more indirections and
adds to the code size.

The old way did clamp the jiffy conversion and thus caused the timeouts
to become negative after some time. Also it didn't work with userspace
which actually fills the upper 32bits of the 64bit timestamp value.
clock_gettime() is 32-bit on 32-bit architectures. Using 64-bit timespec
math, like we do in this commit, means that when a wrap occurs, the
specified timeout goes into the past and we can't request a timeout in
the future. As the Linux implementation of CLOCK_MONOTONIC is reasonable
and starts at 0, the first such timer wrap will occur after approx. 68
years of system uptime.

On GPUs with the security feature the MTLB config is stored in the PTA.
Add a function to trigger the initial PTA load through the FE.

This hooks in the DRM GPU scheduler. No improvement yet, as all the
dependency handling is still done in etnaviv_gem_submit. This just
replaces the actual GPU submit by passing through the scheduler.
Allows to get rid of the retire worker, as this is now driven by the
scheduler.

While the etnaviv workqueue needs to be ordered, as we rely on work items
being executed in queuing order, this is only true for a single GPU.
Having a shared workqueue for all GPUs in the system limits concurrency
artificially.
Getting each GPU its own ordered workqueue still meets our ordering
expectations and enables retire workers to run concurrently.

In order to support performance counters in a sane way we need to provide
a method to sync the GPU with the CPU. The GPU can process multpile command
buffers/events per irq. With the help of a 'sync point' we can trigger an event
and stop the GPU/FE immediately. When the CPU is done with is processing it
simply needs to restart the FE and the GPU will process the command stream.

- process sync point with a work item to keep irq as fast as possible

- call event_free(..) in sync_point_worker(..)

Using the IOMMU API to manage the internal GPU MMU has been an
historical accident and it keeps getting in the way, as well as
entangling the driver with the inner workings of the IOMMU
subsystem.
Clean this up by removing the usage of iommu_domain, which is the
last piece linking etnaviv to the IOMMU subsystem.

Without this, polling on the dma-buf (and presumably other devices
synchronizing against our rendering) would return immediately, even
while the BO was busy.

->fault(), ->page_mkwrite(), and ->pfn_mkwrite() calls do not need to
take a vma and vmf parameter when the vma already resides in vmf.
Remove the vma parameter to simplify things.

Both the safe/scratch address and the master TLB address are per pipe
with the CPU mapped registers not properly propagating to the
different translation units.
The only way to correctly configure all translation units is to have
a command stream snipped executed by the FE, before any other execution
can start.

Currently, we scan the list of mappings each time we want to operate on
the vram_mapping struct. Rather than repeatedly scanning these, look
them up once in the submission path, and then use _reference and
_unreference methods as necessary to manage this object.
