2007-10-10	Drop 'size' argument from bio_endio and bi_end_io	NeilBrown	1	-4/+1
	As bi_end_io is only called once when the reqeust is complete, the 'size' argument is now redundant. Remove it. Now there is no need for bio_endio to subtract the size completed from bi_size. 	blk_rq_bio_prep is exported for use in exactly one place. That place can benefit from using the new blk_rq_append_bio instead. So - change dm-emc to call blk_rq_append_bio - stop exporting blk_rq_bio_prep, and - initialise rq_disk in blk_rq_bio_prep, as dm-emc needs it. 	Right now ->flags is a bit of a mess: some are request types, and others are just modifiers. Clean this up by splitting it into ->cmd_type and ->cmd_flags. 	Initial git repository build. I'm not bothering with the full history, even though we have it. We can create a separate "historical" git archive of that later if we want to, and in the meantime it's about 3.2GB when imported into git - space that would just make the early git days unnecessarily complicated, when we don't have a lot of good infrastructure for it. Let it rip!
