The module is used only as a library for now. Remove module init and exit
routines to show this.
While here, remove FSF snail address and attach EXPORT_SYMBOL() macros to
corresponding functions.

Let's not mix headers used by the core with those headers that
are needed by some driver-specific interface header.
The headers used on drivers were manually moved using:

Split ioctl interface from enum_freq_bands, g_tuner and s_hw_freq_seek
functions and export them to be used in other drivers like bttv.

Since all the drivers that use `struct v4l2_fh' use the core
priority checking, the setting of the flag in the drivers can
be removed.

It seems that, on such arch, the includes from asm/ should be
after the ones from linux/.
The proper fix would be to patch the arch files, but, as
this fix is trivial, apply it. Also, we generally put the
asm includes after the linux ones, anyway.
