Use firmware version field as an indication to health buffer's sanity.
When firmware version is 0xFFFFFFFF, deduce that firmware is unavailable
and avoid printing the health buffer to dmesg as it doesn't provide
debug info.

Firmware logs its asserts also to non-volatile memory. In order to
reduce drift between the NIC and the host, the driver sets the host
epoch-time to the firmware every hour.

Add log macro which gets log level as a parameter. Use the severity
read from the health buffer and the new log macro to log the health buffer
with severity as log level. Prior to this patch, health buffer was
printed in error log level regardless of its severity. Now the user may
filter dmesg (--level) or change kernel log level to focus on different
severity levels of firmware errors.

Enhance health buffer to include:
- assert_var5: expose the 6'th assert variable.
- time: error's time-stamp in seconds (epoch time).
- rfr: Recovery Flow Requiered. When set, indicates that the error
cannot be recovered without flow involving reset.
- severity: error's severity value, ranging from emergency to debug.
Expose them in the health buffer dump (dmesg and devlink fw reporter).

Replace hard coded timeouts with values stored by firmware in default
timeouts register (DTOR). Timeouts are read during driver load. If DTOR
is not supported by firmware then fallback to hard coded defaults
instead.

New mlx5_core device structure is allocated through devlink_alloc
with\ kzalloc and that ensures that all fields are equal to zero
and it includes ->state too.
That means that checks of that field in the mlx5_init_one() is
completely redundant, because that function is called only once
in the begging of mlx5_core_dev lifetime.
PCI:
.probe()
-> probe_one()
-> mlx5_init_one()
The recovery flow can't run at that time or before it, because relevant
work initialized later in mlx5_init_once().
Such initialization flow ensures that dev->state can't be
MLX5_DEVICE_STATE_UNINITIALIZED at all, so remove such impossible
checks.

When fw_fatal reporter reports an error, the firmware in not responding.
Unload the device to ensure that the driver closes all its resources,
even if recovery is not due (user disabled auto-recovery or reporter is
in grace period). On successful recovery the device is loaded back up.

MLX5_INTERFACE_STATE_UP is far from being reliable check for success to
recover, because it can be changed any time and health logic doesn't
have any locks to protect from it.
The locks are not needed here because health recover is good to have,
but not must to success, so rely on the returned value from the
mlx5_recover_device() as a marker for success/failure.

Currently, when we discover a fatal error, we are queueing a work that
will wait for a lock in order to enter the device to error state.
Meanwhile, FW commands are still being processed, and gets timeouts.
This can block the driver for few minutes before the work will manage
to get the lock and enter to error state.
Setting the device to error state before queueing health work, in order
to avoid FW commands being processed while the work is waiting for the
lock.

Once the driver gets sync_reset_request from firmware it prepares for the
coming reset and sends acknowledge.
After getting this event the driver expects device reset, either it will
trigger PCI reset on sync_reset_now event or such PCI reset will be
triggered by another PF of the same device. So it moves to reset
requested mode and if it gets PCI reset triggered by the other PF it
detect the reset and reloads.

Currently, in case of fatal error during mlx5_load_one(), we cannot
enter error state until mlx5_load_one() is finished, what can take
several minutes until commands will get timeouts, because these commands
can't be processed due to the fatal error.
Fix it by setting dev->state as MLX5_DEVICE_STATE_INTERNAL_ERROR before
requesting the lock.

When health reporter is registered to devlink, devlink will implicitly set
auto recover if and only if the reporter has a recover method. No reason
to explicitly get the auto recover flag from the driver.
Remove this flag from all drivers that called
devlink_health_reporter_create.
All existing health reporters set auto recovery to true if they have a
recover method.
Yet, administrator can unset auto recover via netlink command as prior to
this patch.

In mlx5_fw_fatal_reporter_dump if mlx5_crdump_collect fails the
allocated memory for cr_data must be released otherwise there will be
memory leak. To fix this, this commit changes the return instruction
into goto error handling.

When fw fatal error occurs, poll health() first detects and reports on a
fw error. Afterwards, it detects and reports on the fw fatal error
itself.
That can cause a long delay in fw fatal error handling which waits in a
queue for the fw error handling to be finished. The fw error handle will
try asking for fw core dump command while fw in fatal state may not
respond and driver will wait for command timeout.
Changing the flow to detect and handle first fw fatal errors and only if
no fatal error detected look for a fw error to handle.

Crdump repeats itself every chunk of 256bytes.
That is due to bug of missing progressing offset while copying the data
from buffer to devlink_fmsg.

Report devlink health on FW fatal issues via fw_fatal_reporter. The
driver recover flow for FW fatal error is now being handled by the
devlink health.
Having the recovery controlled by devlink health, the user has the
ability to cancel the auto-recovery for debug session and run it
manually.
Call mlx5_enter_error_state() before calling devlink_health_report() to
ensure entering device error state even if auto-recovery is off.

Add support of dump callback for mlx5 FW fatal reporter.
The FW fatal dump uses cr-dump functionality to gather cr-space data for
debug. The cr-dump uses vsc interface which is valid even if the FW
command interface is not functional, which is the case in most FW fatal
errors.

Create mlx5_devlink_health_reporter for fw fatal reporter.
The fw fatal reporter is added in addition to the fw reporter and
implements the recover callback.
The point of having two reporters for FW issues, is that we
don't want to run FW recover on any issue, but only fatal ones.

Use devlink_health_report() to report any symptom of FW issue as FW
counter miss or new health syndrome.
The FW issues detected in mlx5 during poll_health which is called in
timer atomic context and so health work queue is used to schedule the
reports.

Add support of dump callback for mlx5 FW reporter. Once we trigger FW
dump, the FW will write the core dump to its raw data buffer. The tracer
translates the raw data to traces and save it to a cyclic array. Once
dump is done, the saved traces data is filled into the dump buffer. In
case syndrome is not zero the health buffer content will be printed as
well.

Create mlx5_devlink_health_reporter for FW reporter. The FW reporter
implements devlink_health_reporter diagnose callback.
The fw reporter diagnose command can be triggered any time by the user
to check current fw status.
In healthy status, it will return clear syndrome. Otherwise it will
return the syndrome and description of the error type.
Command example and output on healthy status:

If a FW assert is considered fatal, indicated by a new bit in the health
buffer, reset the FW. After the reset go through the normal recovery
flow. Only one PF needs to issue the reset, so an attempt is made to
prevent the 2nd function from also issuing the reset.
It's not an error if that happens, it just slows recovery.

New mlx5 adapters allow the driver to reset the FW in the event of an
error, this action called "SW Reset". When an SW reset is issued on any
PF all PFs enter reset state which is a recoverable condition. The
existing recovery flow was designed to allow the recovery of a VF after
a PF driver reload. This patch adds the sw reset to the NIC states
as a preparation for sw reset handling.
When a software reset is issued the following occurs:
1. The NIC interface mode is set to 7 while the reset is in progress.
2. Once the reset completes the NIC interface mode is set to 1.

Currently mlx5 core stores copy of the PCI device name in a
mlx5_priv structure and uses pr_warn, pr_err helpers.
Get rid of the copy of this name; instead store the parent device
pointer that contains name as well as dma specific parameters.
This also allows to use kernel's well defined dev_warn, dev_err, dev_dbg
device specific print routines.
This is also a preparation patch to access non PCI parent device in
future.

to provide a better report/debug of different mlx5 device types.

Software structure initialization should be in mdev_init stage.
This provides a better logical separation of mlx5 core device
initialization flow and will help to seamlessly support creating different
mlx5 device types such as PF, VF and SF mlx5 sub-function virtual device.
This patch does not change any functionality.

When EEH is injected and PCI bus stalls, mlx5's pci error detect
function is called to deactivate the command interface and tear down
the device. The issue is that there can be a thread that already
passed MLX5_DEVICE_STATE_INTERNAL_ERROR check, it will send the command
and stuck in the wait_func.
Solution:
Add function mlx5_cmd_flush to disable command interface and clear all
the pending commands. When device state is set to
MLX5_DEVICE_STATE_INTERNAL_ERROR, call mlx5_cmd_flush to ensure all
pending threads waiting for firmware commands completion are terminated.

The mlx5_interface->event callback is not used by mlx5e/mlx5_ib anymore.
We totally remove the delayed events logic work around, since with
the dynamic notifier registration API it is not needed anymore, mlx5_ib
can register its notifier and start receiving events exactly at the moment
it is ready to handle them.

In mlx5 we have three types of usages for EQs,
1. Asynchronous EQs, used internally by mlx5 core for
a. FW command completions
b. FW page requests
c. one EQ for all other Asynchronous events
2. Completion EQs, used for CQ completion (we create one per core)
3. *Special type of EQ (page fault) used for RDMA on demand paging
(ODP).
*The 3rd type shouldn't be special at least in mlx5 core, it is yet
another async events EQ with specific use case, it will be removed in
the next two patches, and will completely move its logic to mlx5_ib,
as it is rdma specific.
In this patch we remove use case (eq type) specific fields from
struct mlx5_eq into a new eq type specific structures.
struct mlx5_eq_async;
truct mlx5_eq_comp;
struct mlx5_eq_pagefault;
Separate between their type specific flows.
In the future we will allow users to create there own generic EQs.
for now we will allow only one for ODP in next patches.
We will introduce event listeners registration API for those who
want to receive mlx5 async events.
After that mlx5 eq handling will be clean from feature/user specific
handling.

Move unnecessary EQ table structures and declaration from the
public include/linux/mlx5/driver.h into the private area of mlx5_core
and into eq.c/eq.h.
Introduce new mlx5 EQ APIs:
mlx5_comp_vectors_count(dev);
mlx5_comp_irq_get_affinity_mask(dev, vector);
And use them from mlx5_ib or mlx5e netdevice instead of direct access to
mlx5_core internal structures.

Today mlx5 devices support two teardown modes:
1- Regular teardown
2- Force teardown
This change introduces the enhanced version of the "Force teardown" that
allows SW to perform teardown in a faster way without the need to reclaim
all the pages.
Fast teardown provides the following advantages:
1- Fix a FW race condition that could cause command timeout
2- Avoid moving to polling mode
3- Close the vport to prevent PCI ACK to be sent without been scatter
to memory

When the mlx5 health mechanism detects a problem while the driver
is in the middle of init_one or remove_one, the driver needs to prevent
the health mechanism from scheduling future work; if future work
is scheduled, there is a problem with use-after-free: the system WQ
tries to run the work item (which has been freed) at the scheduled
future time.
Prevent this by disabling work item scheduling in the health mechanism
when the driver is in the middle of init_one() or remove_one().

All other mlx5_events report the port number as 1 based, which is how FW
reports it in the port event EQE. Reporting 0 for this event causes
mlx5_ib to not raise a fatal event notification to registered clients
due to a seemingly invalid port.
All switch cases in mlx5_ib_event that go through the port check are
supposed to set the port now, so just do it once at variable
declaration.
