Pull core block layer updates from Jens Axboe:
"This is the pull request for the core block IO bits for the 3.15
kernel. It's a smaller round this time, it contains:
- Various little blk-mq fixes and additions from Christoph and
myself.
- Cleanup of the IPI usage from the block layer, and associated
helper code. From Frederic Weisbecker and Jan Kara.
- Duplicate code cleanup in bio-integrity from Gu Zheng. This will
give you a merge conflict, but that should be easy to resolve.
- blk-mq notify spinlock fix for RT from Mike Galbraith.

As result deprecation of MSI-X/MSI enablement functions
pci_enable_msix() and pci_enable_msi_block() all drivers
using these two interfaces need to be updated to use the
new pci_enable_msi_range() and pci_enable_msix_range()
interfaces.

If, when the ipr driver loads, the adapter is in an EEH error state,
it will currently oops and not be able to recover, as it attempts
to access memory that has not yet been allocated. We've seen this
occur in some kexec scenarios. The following patch fixes the oops
and also allows the driver to recover from these probe time EEH errors.

This was a debugging measure to toggle enabled/disabled
when testing. But for real production setups, it's not
safe to toggle this setting without either reloading
drivers of quiescing IO first. Neither of which the toggle
enforces.
Additionally, it makes drivers deal with the conditional
state.
Remove it completely. It's up to the driver whether iopoll
is enabled or not.

Some host adapters do not pass commands through to the target disk
directly. Instead they provide an emulated target which may or may not
accurately report its capabilities. In some cases the physical device
characteristics are reported even when the host adapter is processing
commands on the device's behalf. This can lead to adapter firmware hangs
or excessive I/O errors.
This patch disables WRITE SAME for devices connected to host adapters
that provide an emulated target. Driver writers can disable WRITE SAME
by setting the no_write_same flag in the host adapter template.

The 'ctl' field of the 'struct ata_taskfile' is not really dual purpose, i.e.
it is not intended for storing the alternate status register (which is mapped
at the same address in the legacy IDE controllers) in the qc_fill_rtf() method.
No other 'libata' driver except 'drivers/scsi/ipr.c' stores the alternate status
register's value in the 'ctl' field of 'qc->result_tf', hence this driver should
not do this as well...

Reinitialize resource queue prior to freeing resource entries to ensure they

The abort command issued by ipr_cancel_op() is being added to the wrong
HRRQ free queue after the command returns. Fix it by using the HRRQ
pointer in the ipr command struct itself.

Loading ipr modules failed(-22) with msi-x enabled adapter. In ipr_test_msi(),
We need to pass the first vector of msix vectors instead of using pdev->irq
to request_irq() when adapter enables msix feature.

When resetting an ipr adapter, we use scsi_block_requests to
block any new commands from scsi core, and then unblock after
the reset. When hotplug removing an adapter, we shut it down
and go through this same code, but we've seen issues with
scsi_unblock_requests running after the adapter's memory has
been freed. There is really no need to block/unblock when
the adapter is being removed, so this patch skips the
block/unblock and will immediately fail any commands that
happen to make it to queuecommand while the adapter is
being shutdown.

The resource path as displayed by the ipr driver is the
location string identifying a location on the SAS fabric.
This patch adds the SCSI host number such that error logs
can be more easily correlated in multiple adapter configurations.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.
This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.
Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.

Pull workqueue changes from Tejun Heo:
"This is workqueue updates for v3.7-rc1. A lot of activities this
round including considerable API and behavior cleanups.
* delayed_work combines a timer and a work item. The handling of the
timer part has always been a bit clunky leading to confusing
cancelation API with weird corner-case behaviors. delayed_work is
updated to use new IRQ safe timer and cancelation now works as
expected.
* Another deficiency of delayed_work was lack of the counterpart of
mod_timer() which led to cancel+queue combinations or open-coded
timer+work usages. mod_delayed_work[_on]() are added.
These two delayed_work changes make delayed_work provide interface
and behave like timer which is executed with process context.
* A work item could be executed concurrently on multiple CPUs, which
is rather unintuitive and made flush_work() behavior confusing and
half-broken under certain circumstances. This problem doesn't
exist for non-reentrant workqueues. While non-reentrancy check
isn't free, the overhead is incurred only when a work item bounces
across different CPUs and even in simulated pathological scenario
the overhead isn't too high.
All workqueues are made non-reentrant. This removes the
distinction between flush_[delayed_]work() and
flush_[delayed_]_work_sync(). The former is now as strong as the
latter and the specified work item is guaranteed to have finished
execution of any previous queueing on return.
* In addition to the various bug fixes, Lai redid and simplified CPU
hotplug handling significantly.
* Joonsoo introduced system_highpri_wq and used it during CPU
hotplug.
There are two merge commits - one to pull in IRQ safe timer from
tip/timers/core and the other to pull in CPU hotplug fixes from
wq/for-3.6-fixes as Lai's hotplug restructuring depended on them."
Fixed a number of trivial conflicts, but the more interesting conflicts
were silent ones where the deprecated interfaces had been used by new
code in the merge window, and thus didn't cause any real data conflicts.

We have an old FIXME in reg.h which points out that we should standardise
on PVR_foo for our PVR #defines. Currently we use PVR_ on 32-bit and PV_
on 64-bit.
So do that rename and remove the FIXME.
Seeing as we're touching all but one usage of __is_processor(), rename it
to something less ugly and more indicative of what it does, which is
simply to check the PVR version.

libsas and ipr pass flags to ata_host_init that are meant for the port.
ata_host flags:

flags passed by libsas:

The only one that aliases is ATA_HOST_STARTED which is a 'don't care' in
the libsas and ipr cases since ata_hosts from these sources are not
registered with libata.

flush[_delayed]_work_sync() are now spurious. Mark them deprecated
and convert all users to flush[_delayed]_work().
If you're cc'd and wondering what's going on: Now all workqueues are
non-reentrant and the regular flushes guarantee that the work item is
not pending or running on any CPU on return, so there's no reason to
use the sync flushes at all and they're going away.
This patch doesn't make any functional difference.

This changes the ordering of initialization and probing events from:
1/ allocate rphy in PORTE_BYTES_DMAED, DISCE_REVALIDATE_DOMAIN
2/ allocate ata_port and schedule port probe in DISCE_PROBE
...to:
1/ allocate ata_port in PORTE_BYTES_DMAED, DISCE_REVALIDATE_DOMAIN
2/ allocate rphy in PORTE_BYTES_DMAED, DISCE_REVALIDATE_DOMAIN
3/ schedule port probe in DISCE_PROBE
This ordering prevents PHYE_SIGNAL_LOSS_EVENTS from sneaking in to
destrory ata devices before they have been fully initialized:

The latest generation of ipr hardware performs best when command blocks
are aligned to a boundary equal to the size of the command block. Ensure
512 byte alignment, since this is the largest size command block we
can send.

The ipr driver added some memory barriers in order to ensure
a PowerPC sync instruction was executed prior to sending a
command to the adapter to ensure the command block was
coherent with respect to the PCI bus's view of memory.
However, some time ago, the powerpc architecture writel
macros were changed to include the sync since most drivers
don't properly handle this. So remove these memory barriers
since they are not needed and result in executing twice
as many sync instructions, which has a significant performance
penalty.

to clear the interrupt so remove this to optimize performance.

For the latest ipr SAS adapters, target id's are a completely
logical construct that are managed in the ipr driver. This fixes
an issue that can arise if a device is deleted via sysfs. If
a new device is then physically added, it will use the previous
device's target id. If the host is then rescanned, the device
that had been deleted, since it is using the same target id as
the new device is using, will never be found, resulting in
a missing device. Fix this by only freeing the target id
only if the resource is actually gone.
