Merge phy fixes for 5.17 into next branch to resolve the conflict

Due to historical reasons sclp_sdias_copy() misuses
the notion of physical vs virtual addresses difference.
Note: this does not fix a bug currently, since virtual
and physical addresses are identical.

This patch adds s390dbf logging to the function that executes the
PQAP(AQIC) instruction on behalf of the guest to which the queue for which
interrupts are being enabled or disabled is attached.
Currently, the vfio_ap_irq_enable function sets status response code 06
(notification indicator byte address (nib) invalid) in the status word
when the vfio_pin_pages function - called to pin the page containing the
nib - returns an error or a different number of pages pinned than
requested.
Setting the response code returned to userspace without also logging a
message in the kernel makes it impossible to determine whether the response
was due to an error detected by the vfio_ap device driver or because the
response code was returned by the firmware in response to the PQAP(AQIC)
instruction.
In addition to logging a warning for the situation above, this patch adds
the following:
* A function to validate the nib address invoked prior to calling the
vfio_pin_pages function. This allows for logging a message informing
the reader of the reason the page containing the nib can not be pinned
if the nib address is not valid. Response code 06 (invalid nib address)
will be set in the status word returned to the guest from the
instruction.
* Checks the return value from the kvm_s390_gisc_register and logs a
message informing the reader of the failure. Status response code 08
(invalid gisa) will be set in the status word returned to the guest from
the PQAP(AQIC) instruction.
* Checks the status response code returned from execution of the PQAP(AQIC)
instruction and if it indicates an error, logs a message informing the
reader.

This patch adds s390dbf logging to the function that handles interception
of the PQAP(AQIC) instruction. Several items of data are validated before
ultimately calling the functions that execute the PQAP(AQIC) instruction on
behalf of the guest to which the queue for which interrupts are being
enabled or disabled is attached.
Currently, the handle_pqap function sets status response code 01 (queue not
available) in the status word that is normally returned from the
PQAP(AQIC) instruction under the following conditions:
* Set when the function pointer to the handler is not set in the
kvm_s390_crypto object (i.e., the PQAP hook is not registered).
* Set when the KVM pointer is not set in the ap_matrix_mdev object
(i.e., the matrix mdev is not passed through to a guest).
* Set when the queue for which interrupts are being enabled or
disabled is either not bound to the vfio_ap device driver or not assigned
to the matrix mdev.
Setting the response code returned to userspace without also logging a
message in the kernel makes it impossible to determine whether the response
was due to an error detected by the vfio_ap device driver or because the
response code was returned by the firmware in response to the PQAP(AQIC)
instruction, so this patch logs a message to the s390dbf log for the
vfio_ap device driver for each of the situations described above.

When setting up autonegotiation for 88E1118R and compatible PHYs,
a software reset of PHY is issued before setting up polarity.
This is incorrect as changes of MDI Crossover Mode bits are
disruptive to the normal operation and must be followed by a
software reset to take effect. Let's patch m88e1118_config_aneg()
to fix the issue mentioned before by invoking software reset

Instead of disabling TSO at compile time if MAX_SKB_FRAGS > 32,
implement ndo_features_check() method for this driver for
a more dynamic handling.
If skb has more than 32 frags and is a GSO packet, force
software segmentation.
Most locally generated packets will use a small number
of fragments anyway.
For forwarding workloads, we can limit gro_max_size at ingress,
we might also implement gro_max_segs if needed.

It seems this one-element array is not actually being used as an
array of variable size, so we can just replace it with just a
non-array object of type struct desc_frag and refactor a bit the
rest of the code.
This helps with the ongoing efforts to globally enable -Warray-bounds
and get us closer to being able to tighten the FORTIFY_SOURCE routines
on memcpy().
This issue was found with the help of Coccinelle and audited and fixed,

There is a regular need in the kernel to provide a way to declare having
a dynamically sized set of trailing elements in a structure. Kernel code
should always use “flexible array members”[1] for these cases. The older
style of one-element or zero-length arrays should no longer be used[2].

The filters for the PTP trap keys are incorrectly configured, in the
sense that is2_entry_set() only looks at trap->key.ipv4.dport or
trap->key.ipv6.dport if trap->key.ipv4.proto or trap->key.ipv6.proto is
set to IPPROTO_TCP or IPPROTO_UDP.
But we don't do that, so is2_entry_set() goes through the "else" branch
of the IP protocol check, and ends up installing a rule for "Any IP
protocol match" (because msk is also 0). The UDP port is ignored.
This means that when we run "ptp4l -i swp0 -4", all IP traffic is
trapped to the CPU, which hinders bridging.
Fix this by specifying the IP protocol in the VCAP IS2 filters for PTP
over UDP.

In case the entropy quality is low, there may be less than one bit to
credit in the call to add_hwgenerator_randomness(): The number of bytes
returned by rng_get_data() multiplied by the current quality (in entropy
bits per 1024 bits of input) must be larger than 128 to credit at least
one bit. However, imx-rngc.c sets the quality to 19, but may return less
than 32 bytes; hid_u2fzero.c sets the quality to 1; and users may override
the quality setting manually.
In case there is less than one bit to credit, keep track of it and add
that credit to the next iteration.

LDWB is getting incorrectly used in HW when
CPT_AF_LF()_PTR_CTL[IQB_LDWB]=1 and CPT instruction queue has less than
320 free entries. So, increase HW instruction queue size by 320 and give
320 entries less for SW/NIX RX as a SW workaround.

When CPT_AF_DIAG[FLT_DIS] = 0 and a CPT engine access to
LLC/DRAM encounters a fault/poison, a rare case may result
in unpredictable data being delivered to a CPT engine.
So, this patch adds code to set FLT_DIS as a workaround.
