Create Documentation/blockdev/ sub-directory and populate it.
Populate the Documentation/serial/ sub-directory.
Move MSI-HOWTO.txt to Documentation/PCI/.
Move ioctl-number.txt to Documentation/ioctl/.
Update all relevant 00-INDEX files.
Update all relevant Kconfig files and source files.

Since the "ramdisk" kernel parameter has been officially deprecated
since at least 2.6.18, might as well finally get rid of it.

The default ramdisk blocksize is actually 1024, not 512 bytes.  Also fixes
up some trailing whitespace issues.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
