Use Joe Perches cvt_fallthrough.pl script to convert

	/* fallthrough */

comments (and its derivatives) into a

	fallthrough;

statement. This automatically drops useless ones.

Do it MTD-wide.

Use DIV_ROUND_UP() in cfi_udelay() instead of open-coding it...

Doing this also helpfully gets rid of two complaints from
'scripts/checkpatch.pl --strict':

In preparation to enabling -Wimplicit-fallthrough, mark switch

With this .config: http://busybox.net/~vda/kernel_config,
after uninlining these functions have sizes and callsite counts
as follows:

cfi_udelay(): 74 bytes, 26 callsites
cfi_send_gen_cmd(): 153 bytes, 95 callsites
cfi_build_cmd(): 274 bytes, 123 callsites
cfi_build_cmd_addr(): 49 bytes, 15 callsites
cfi_merge_status(): 230 bytes, 3 callsites

Reduction in code size is about 50,000:

We already verify that offset and length are within the MTD device size
in the MTD API functions. Let's remove the duplicated checks in drivers.
This patch only affects the following API's:

'mtd_erase()'
'mtd_point()'
'mtd_unpoint()'
'mtd_get_unmapped_area()'
'mtd_read()'
'mtd_write()'
'mtd_panic_write()'
'mtd_lock()'
'mtd_unlock()'
'mtd_is_locked()'
'mtd_block_isbad()'
'mtd_block_markbad()'

This patch adds a bit of noise by removing too sparse empty lines, but this is
not too bad.
