Move platform independent header files to arch/arm/include/asm, leaving
those in asm/arch* and asm/plat* alone.

Make the contents of the userspace asm/setup.h header consistent on all
architectures:

 - export setup.h to userspace on all architectures
 - export only COMMAND_LINE_SIZE to userspace
 - frv: move COMMAND_LINE_SIZE from param.h
 - i386: remove duplicate COMMAND_LINE_SIZE from param.h
 - arm:
   - export ATAGs to userspace
   - change u8/u16/u32 to __u8/__u16/__u32

nommu does not require the page table manipulation code in the
bootmem initialisation paths.  Move this into separate inline
functions.

The `make buildcheck` is erroneously reporting that the earlyparam
list is referencing items in the .init section as it is not itself
postfixed with .init
Also, as per rmk's suggestion, rename the __early_param to
.early_param to bring it into line with everything else

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
