KVM: nSVM: Split off common definitions for Hyper-V on KVM and KVM on Hyper-V
In preparation to implementing Enlightened MSR-Bitmap feature for Hyper-V on KVM, split off the required definitions into common 'svm/hyperv.h' header. No functional change intended.
