[ALSA] Unregister device files at disconnection
Orignally proposed by Sam Revitch <sam.revitch@gmail.com>. Unregister device files at disconnection to avoid the futher accesses. Also, the dev_unregister callback is removed and replaced with the combination of disconnect + free.
