This used to be the default if the lock pointer was set, but now that lock is by
default only used for ioctl serialization. Those drivers that already used
core locking have this flag set explicitly, except for some drivers where
it was obvious that there was no need to serialize any file operations other
than ioctl.
The drivers that didn't need this flag were:

The other drivers that use core locking and where it was not immediately
obvious that this flag wasn't needed were changed so that the flag is set
together with a comment that that driver needs work to avoid having to
set that flag. This will often involve taking the core lock in the fops
themselves.
Eventually this flag should go and it should not be used in new drivers.
There are a few reasons why we want to avoid core locking of non-ioctl
fops: in the case of mmap this can lead to a deadlock in rare situations
since when mmap is called the mmap_sem is held and it is possible for
other parts of the code to take that lock as well (copy_from_user()/copy_to_user()
perform a down_read(&mm->mmap_sem) when a page fault occurs).
It is very unlikely that that happens since the core lock serializes all
fops, but the kernel warns about it if lock validation is turned on.
For poll it is also undesirable to take the core lock as that can introduce
increased latency. The same is true for read/write.
While it was possible to make flags or something to turn on/off taking the
core lock for each file operation, in practice it is much simpler to just
not take it at all except for ioctl and leave it to the driver to take the
lock. There are only a handful fops compared to the zillion ioctls we have.
I also wanted to make it obvious which drivers still take the lock for all
fops, so that's why I chose to have drivers set it explicitly.

In cx231xx_v4l2_close() there are two calls to
cx231xx_release_resources(dev) followed by kfree(dev). That is a
problem since cx231xx_release_resources() already kfree()'s its
argument, so we end up doing a double free.
Easily resolved by just removing the redundant kfree() calls after the
calls to cx231xx_release_resources().
I also changed the 'dev = NULL' assignments (which are rather
pointless since 'dev' is about to go out of scope), to 'fh->dev = NULL'
since it looks to me that that is what was actually intended.
And I removed the 'dev = NULL' assignment at the end of
cx231xx_release_resources() since it is pointless.

The driver were using DEV_MISCONFIGURED on some places, and
DEV_DISCONNECTED on others. In a matter of fact, DEV_MISCONFIGURED
were set only during the usb disconnect callback, with
was confusing.
Also, the alsa driver never checks if the device is present,
before doing some dangerous things.
Remove DEV_MISCONFIGURED, replacing it by DEV_DISCONNECTED.
Also, fixes the other usecases for DEV_DISCONNECTED.

Make use of the signal state registers to properly populate the signal lock
registers in the cx231xx driver.
This allows applications to know whether there is a signal present even in
devices which lack a tuner (since such apps typically won't call G_TUNER if
no tuner is present).

All the modified drivers didn't have any version increment since
Jan, 1 2011. Several of them didn't have any version increment
for a long time, even having new features and important bug fixes
happening.
As we're now filling the QUERYCAP version with the current Kernel
Release, we don't need to maintain a per-driver version control
anymore. So, let's just use the default.
In order to preserve the Kernel module version history, a
KERNEL_VERSION() macro were added to all modified drivers, and
the extraver number were incremented.
I opted to preserve the per-driver version control to a few
pwc, pvrusb2, s2255, s5p-fimc and sh_vou.
A few drivers are still using the legacy way to handle ioctl's.
So, we can't do such change on them, otherwise, they'll break.
Those are: uvc, et61x251 and sn9c102.
The rationale is that the per-driver version control seems to be
actively maintained on those.
Yet, I think that the better for them would be to just use the
default version numbering, instead of doing that by themselves.
While here, removed a few uneeded include linux/version.h

Instead of per-model tests all over the code, use some parameters
at the board entries to describe the model variants for:
- devices with 417 MPEG encoder;
- devices that use external AV;
- devices where vbi VANC endpoint doesn't work;
- devices with xc5000 that require different IF
initialization (and probably will cover also
xc3028).
- devices with xceive tuner that require a reset
during init.

Instead of doing its own lock, use core-assisted one. As a bonus, it
will do the proper unlock during queue wait events. This fixes a
long-standing bug where softwares like tvtime would hang if you try to
use cx231xx-alsa.

Add initial support for the Hauppauge USBLive 2 (2040:c200). Note that I
had to copy a bunch of the case statements used for the Conexant video grabber
reference design (which also doesn't have a tuner). This will likely need to
be refactored out into the board profile.

If switching to a tuner input, reset the standard and tune to the last known
frequency. We need to do this in particular for this bridge since the tuner
gets powered down when captuing on the composite or s-video inputs.

Remove a hack which was jammed into s_input to force the device into bulk mode
at runtime (an abuse of the API). If this sort of functionality is actually
needed (aside from the existing "transfer_mode" modprobe variable), a patch
can be submitted which makes use of a private control.

You cannot call the video_mux routine from within the isoc setup, because that
code is shared with the digital isoc handler. This was causing the GPIOs and
power control to be put into the wrong state when starting up digital mode.

Move the responsibility for setting up the horizontal and vertical scalers
entirely to the cx25840 driver. The cx231xx-avcore was actually programming
garbage into the HSCALE_CTRL and VSCALE_CTRL registers (because of differences
in how the em28xx driver worked, which the cx231xx driver was derived from).
The net effect is that the scaler now works properly (tested with both PAL
and NTSC under mplayer and tvtime).
This patch also gets rid of cx25840 errors showing up in dmesg which say
"720x480 is not a valid size" (since we now properly setup the size of the
active video area).

Add an ext_lock argument to the videobuf init functions. This allows
drivers to pass the vdev->lock pointer (or any other externally held lock)
to videobuf. For now all drivers just pass NULL.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Instead of using the minor number in kernel log messages, use the device
node name as returned by the video_device_node_name() function. This
makes debug, informational and error messages easier to understand for
end users.

Fix all device drivers to use the video_drvdata function instead of
maintaining a local list of minor to private data mappings. Call
video_set_drvdata to register the driver private pointer when not
already done.
Where applicable, the local list of mappings is completely removed when
it becomes unused.

Upcoming I2C v4l2_subdev drivers need a way to control the subdevice
power state from the core. This use case is already partially covered by
the tuner s_standby operation, but no way to explicitly come back from
the standby state is available.
Rename the tuner s_standby operation to core s_power, and fix tuner
drivers accordingly. The tuner core will call s_power(0) instead of
s_standby(). No explicit call to s_power(1) is required for tuners as
they are supposed to wake up from standby automatically.

In the TRY_FMT handler the function get_scale() is called to find what the
scaler hardware will produce for a requested size.
The problem is that get_scale(struct cx231xx *dev, ..., unsigned int *vscale,
unsigned int *hscale) saves the calculated scale values into both the
pointer arguments and into dev's hscale and vscale fields. TRY_FMT shouldn't
actually change anything in the device state.
The code to in get_scale() that writes to dev->[hv]scale can just be
deleted. In all cases when dev's fields should be modified, get_scale()
was called with get_scale(dev, ..., &dev->hscale, &dev->vscale), so dev was
getting updated anyway.
This didn't actually cause a problem because nothing ever actually made use
of the hscale and vscale fields. I changed cx231xx_resolution_set() to use
those fields rather than re-calculate them with a call to get_scale().
Updating [hv]scale in cx231xx_resolution_set() isn't necessary because
every call of cx231xx_resolution_set() was already preceded by a call to
get_scale() or setting the [hv]scale fields, so they will be always be
up-to-date w.r.t. width and height.
Removing the call to get_scale() from cx231xx_resolution_set() allowed
making get_scale() a static function, which is a good thing for something
with such a short name. There is already another function with the same
name in the em28xx driver, but that one is static.

It is no longer needed to use a struct pointer as argument, since v4l2_subdev
doesn't require that ioctl-like approach anymore. Instead just pass the input,
output and config (new!) arguments directly.

s_std didn't belong in the tuner ops. Stricly speaking it should be part of
the video ops, but it is used by audio and tuner devices as well, so it is
more efficient to make it part of the core ops.

VIDIOC_QUERYCAP should return the proper bus info. In the case of USB, this
should be generated by usb_make_path(), being something like:
usb-0000:00:1d.7-2

This patch converts cx231xx to the new v4l2 dev/subdev, doing:
- Conversion of i2c calls to subdev calls;
- all subdev calls to call_all();
- Corrected the header file order in cx231xx.h;
- Added tuner frequency setting.

Only cx231xx-video needs linux/version.h, due to KERNEL_VERSION macro,
that is used by V4L2 API.
This patch moves the KERNEL_VERSION to its proper place and starts with
0,0,1.
There are still much more to be fixed on later patches
