Add support for setting NPPS. NPPS is currently available in
REAL_TIME_CLOCK mode only. In addition allow the user to set the pulse
duration.
When NPPS pulse duration is not set explicitly by the user, driver set
it to 50% of the NPPS period.

When in Real-time mode, HW clock is synced with the PTP daemon. Hence
driver should not re-calibrate the next pulse (via MTPPSE repetitive
events mechanism).
This patch arms repetitive events only in free-running mode.

Allow configuration of 1PPS start time only with time-stamp representing
a round second. Prior to this patch driver allowed setting of a
non-round-second which is not supported by the device. Avoid unexpected
behavior by restricting start-time configuration to a round-second.

Device timestamp can be in real time mode (cycles to time translation is
offloaded into the Hardware). With real time mode, HW provides timestamp
which is already translated into nanoseconds.
With this mode, driver adjusts both the HW and timecounter (to keep
clock_info_page updated) using callbacks: adjfreq, adjtime and settime.
HW clock modifications are done via MTUTC access reg commands. Driver is
allowed to modify HW real time clock only if MCAM ptpcyc2realtime_modify
capability is set.
Add MTUTC set function to be used for configuring the HW real time
clock. Modify existing code to support both internal timer (with
conversion via timecounter_cyc2time() and real time (no conversions).
Align the signatures of the helpers converting from timestamp to
nanoseconds. With that, when allocating a queue assign the corresponding
callback with respect to the capability.
Adjust 1PPS timestamp calculation flows based on the timestamp mode.
Cyc2time offload brings two major advantages:
- Improve MTAE (Max Time Absolute Error) for HW TS by up to 160 ns over a
100% loaded CPU.
- Faster data-path timestamp to nanoseconds, as translation is
lock-less and done in HW.
On real time mode, timestamp format is 32 high bits of seconds and 32
low bits of nanoseconds. On some flows, driver shall convert this format
into nanoseconds wall-clock with REAL_TIME_TO_NS macro.
HW supports a single clock, and it is shared by all functions on a
device. In case real time clock is used, it is recommended to use
a single GM to all device's functions.

Some of PPS logic (timestamp calculations) fits only internal timer
timestamp mode. Move these logics into helper functions. Later in the
patchset cyc2time HW translation mode will expose its own PPS timestamp
calculations.
With this change, main flow will only hold calling PPS logic based on run
time mode.

Internal timer mode (SW clock) requires some PTP clock related metadata
structs. Real time mode (HW clock) will not need these metadata structs.
This separation emphasize the different interfaces for HW clock and SW
clock.

Clock struct is part of struct mlx5_core_dev. Code was inconsistent, on
some cases used container_of and on another used clock->mdev.

In a special configuration, a ConnectX6-Dx pin pps-out might be activated
when driver is loaded. Fix the driver to always read the operational pin
mode when registering it, and advertise it accordingly.

On PTP mlx5_ptp_enable(on=0) flow, driver mistakenly used channel index
as pin index.
After ptp patch marked in fixes tag was introduced, driver can freely
call ptp_find_pin() as part of the .enable() callback.
Fix driver mlx5_ptp_enable(on=0) flow to always use ptp_find_pin(). With
that, Driver will use the correct pin index in mlx5_ptp_enable(on=0) flow.
In addition, when initializing the pins, always set channel to zero. As
all pins can be attached to all channels, let ptp_set_pinfunc() to move
them between the channels.
For stable branches, this fix to be applied only on kernels that includes
both patches in fixes tag. Otherwise, mlx5_ptp_enable(on=0) will be stuck
on pincfg_mux.

User space may request time stamps on rising edges, falling edges, or
both. However, the particular mode may or may not be supported in the
hardware or in the driver. This patch adds a "strict" flag that tells
drivers to ensure that the requested mode will be honored.

Fix the mlx5 core PTP support to explicitly reject any future flags that
get added to the external timestamp request ioctl.
In order to maintain currently functioning code, this patch accepts all
three current flags. This is because the PTP_RISING_EDGE and
PTP_FALLING_EDGE flags have unclear semantics and each driver seems to
have interpreted them slightly differently.
[ RC: I'm not 100% sure what this driver does, but if I'm not wrong it
follows the dp83640:

Commit 823eb2a3c4c7 ("PTP: add support for one-shot output") introduced
a new flag for the PTP periodic output request ioctl. This flag is not
currently supported by any driver.
Fix all drivers which implement the periodic output request ioctl to
explicitly reject any request with flags they do not understand. This
ensures that the driver does not accidentally misinterpret the
PTP_PEROUT_ONE_SHOT flag, or any new flag introduced in the future.
This is important for forward compatibility: if a new flag is
introduced, the driver should reject requests to enable the flag until
the driver has actually been modified to support the flag in question.

The timecounter needs to be updated at least once in half of the
cyclecounter interval to prevent timecounter_cyc2time() interpreting a
new timestamp as an old value and causing a backward jump.
This would be an issue if the timecounter multiplier was so small that
the update interval would not be limited by the 64-bit overflow in
multiplication.
Shorten the calculated interval to make sure the timecounter is updated
in time even when the system clock is slowed down by up to 10%, the
multiplier is increased by up to 10%, and the scheduled overflow check
is late by 15%.

Changed "priv.clock.lock" lock from 'rw_lock' to 'seq_lock'
in order to improve packet rate performance.
Tested on Intel(R) Xeon(R) CPU E5-2660 v2 @ 2.20GHz.
Sent 64b packets between two peers connected by ConnectX-5,
and measured packet rate for the receiver in three modes:
no time-stamping (base rate)
time-stamping using rw_lock (old lock) for critical region
time-stamping using seq_lock (new lock) for critical region
Only the receiver time stamped its packets.
The measured packet rate improvements are:
Single flow (multiple TX rings to single RX ring):
without timestamping: 4.26 (M packets)/sec
with rw-lock (old lock): 4.1 (M packets)/sec
with seq-lock (new lock): 4.16 (M packets)/sec
1.46% improvement
Multiple flows (multiple TX rings to six RX rings):
without timestamping: 22 (M packets)/sec
with rw-lock (old lock): 11.7 (M packets)/sec
with seq-lock (new lock): 21.3 (M packets)/sec
82.05% improvement
The packet rate improvement is due to the lack of atomic operations
for the 'readers' by the seq-lock.
Since there are much more 'readers' than 'writers' contention
on this lock, almost all atomic operations are saved.
this results in a dramatic decrease in overall
cache misses.

When driver converts HW timestamp to wall clock time it subtracts
the last saved cycle counter from the HW timestamp and converts the
difference to nanoseconds.
The conversion is done by multiplying the cycles difference with the
clock multiplier value as a first step and therefore the cycles
difference should be small enough so that the multiplication product
doesn't exceed 64bit.
The overflow handling routine is in charge of updating the last saved
cycle counter in driver and it is called periodically using kernel
delayed workqueue.
The delay period for this work is calculated using the max HW cycle
counter value (a 41 bit mask) as a base which doesn't take the 64bit
limit into account so the delay period may be incorrect and too
long to prevent a large difference between the HW counter and the last
saved counter in SW.
This change adjusts the work period for the HW clock overflow work by
taking the minimum between the previous value and the quotient of max
u64 value and the clock multiplier value.

The kzalloc() function has a 2-factor argument form, kcalloc(). This
patch replaces cases of:
kzalloc(a * b, gfp)
with:
kcalloc(a * b, gfp)
as well as handling cases of:
kzalloc(a * b * c, gfp)
with:
kzalloc(array3_size(a, b, c), gfp)
as it's slightly less ugly than:
kzalloc_array(array_size(a, b), c, gfp)
This does, however, attempt to ignore constant size factors like:
kzalloc(4 * 1024, gfp)
though any constants defined via macros get caught up in the conversion.
Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

Adds a new page to mlx5 core containing clock info data that allows
user level applications to translate between cqe timestamp to
nanoseconds. The information stored into this page is represented
through mlx5_ib_clock_info.
In order to synchronize between kernel and user space a sequence
number is incremented at the beginning and end of each update.
An odd number means the data is being updated while an even means
the access was already done. To guarantee that the data structure
was accessed atomically user will:
repeat:
seq1 = <read sequence>
goto <repeate> while odd
<read data structure>
seq2 = <read sequence>
if seq1 != seq2 goto repeat

PPS event did not update ptp_clock_event fields, therefore,
timestamp value was not updated correctly. This fix updates the
event source and the timestamp value for each PPS event.

PTP code is moved to core section of mlx5 driver in order to share
it between ethernet and infiniband. This movement involves the following
changes:
- Change mlx5e_ prefix to be mlx5_
- Add clock structs to Core
- Add clock object to mlx5_core_dev
- Call Init/Uninit clock from core init/cleanup
- Rename mlx5e_tstamp to be mlx5_clock

en_clock.c renamed clock.c and moved to lib/ as first step
towards relocating code to core part of the driver to allow
