2026-09-01scsi: mpi3mr: Fix use-after-free on tgt_dev->starget during target device refresh/updateLaurence Oberman1-10/+35
mpi3mr_refresh_tgtdevs() and mpi3mr_devinfochg_evt_bh() read tgt_dev->starget and immediately pass it to starget_for_each_device() without holding mrioc->tgtdev_lock. Every writer of this field -- mpi3mr_target_alloc(), mpi3mr_target_destroy(), mpi3mr_slave_destroy() and mpi3mr_sdev_init() -- correctly serializes access under tgtdev_lock, but these two read sites do not, which leaves a check-then-use window against the SCSI core's target teardown path (scsi_remove_target(), invoked e.g. via a concurrent host reset, sysfs "delete", or SCSI EH device offlining running independently of the fwevt workqueue). Sequence observed on production hardware, triggered on the mpi3mr0_fwevt_wrkr workqueue during a SAS topology change shortly after a controller reset: BUG: kernel NULL pointer dereference, address: 0000000000000058 RIP: scsi_is_host_device+0x7/0x20 Call Trace: starget_for_each_device+0x34/0x100 mpi3mr_refresh_tgtdevs+0x152/0x1d0 [mpi3mr] mpi3mr_fwevt_bh+0x514/0x6c0 [mpi3mr] mpi3mr_fwevt_worker+0x1a/0x50 [mpi3mr] process_one_work+0x194/0x380 worker_thread+0x2fe/0x410 mpi3mr_refresh_tgtdevs() reads tgt_dev->starget as non-NULL, but by the time starget_for_each_device() dereferences it, a concurrent mpi3mr_target_destroy() has already cleared tgt_dev->starget under tgtdev_lock and the SCSI/device core has freed the underlying scsi_target (and its embedded struct device). The stale pointer is then walked by dev_to_shost() -> scsi_is_host_device(), producing the NULL/garbage dereference above. Fix this by taking mrioc->tgtdev_lock around every read of tgt_dev->starget, matching the existing writer-side discipline. Since starget_for_each_device() and mpi3mr_update_sdev() can end up doing non-atomic work (e.g. queue_limits_commit_update()), the lock cannot be held across the whole call, so instead pin the target's device with get_device() while holding the lock, drop the lock, then run starget_for_each_device() against the pinned reference and put_device() afterwards. This closes the TOCTOU window instead of merely narrowing it. The same unlocked read-and-dereference pattern also exists earlier in mpi3mr_refresh_tgtdevs()'s first removal-scan loop (tgt_dev->starget->hostdata); fix it the same way by holding tgtdev_lock across that check, which is cheap since it only touches plain struct fields. 2026-08-28scsi: mpi3mr: Fix target device refcount leak in mpi3mr_sas_port_add()Milan P. Gandhi1-0/+2
mpi3mr_get_tgtdev_by_addr() increments the target device kref when it returns a device. If a subsequent error triggers a goto out_fail after the tgtdev reference is acquired, the reference is never released because the out_fail path does not call mpi3mr_tgtdev_put(). This prevents the target device structure from ever being freed. Add a tgtdev put in the out_fail path, guarded by a NULL check since tgtdev is only acquired for SAS_END_DEVICE types and the same cleanup path is shared by earlier error cases where tgtdev is still NULL. 2026-08-28scsi: mpi3mr: Fix NULL pointer dereference in mpi3mr_sas_port_add()Milan P. Gandhi1-0/+6
sas_port_alloc_num() can return NULL on memory allocation failure. The return value is passed directly to sas_port_add() without a NULL check, which causes a NULL pointer dereference. Additionally, if sas_port_add() fails, the allocated port is not freed before jumping to out_fail, leaking the sas_port structure. Call sas_port_free() to properly release it. 2026-07-28scsi: mpi3mr: Fix potential deadlock in mpi3mr_fault_uevent_emitChandrakanth Patil1-2/+7
mpi3mr_fault_uevent_emit() runs from the fault watchdog and reset paths where host I/O may already be blocked. GFP_KERNEL allocations here, both the local kzalloc_obj() and the ones inside kobject_uevent_env() itself, can trigger reclaim that waits on that blocked I/O and deadlock. Use memalloc_noio_save()/restore() to cover the whole call instead of just the local allocation. 2026-02-28scsi: mpi3mr: Clear reset history on ready and recheck state after timeoutRanjan Kumar1-0/+10
The driver retains reset history even after the IOC has successfully reached the READY state. That leaves stale reset information active during normal operation and can mislead recovery and diagnostics. In addition, if the IOC becomes READY just as the ready timeout loop exits, the driver still follows the failure path and may retry or report failure incorrectly. Clear reset history once READY is confirmed so driver state matches actual IOC status. After the timeout loop, recheck the IOC state and treat READY as success instead of failing. 2026-02-21Convert more 'alloc_obj' cases to default GFP_KERNEL argumentsLinus Torvalds2-4/+2
This converts some of the visually simpler cases that have been split over multiple lines. I only did the ones that are easy to verify the resulting diff by having just that final GFP_KERNEL argument on the next line. Somebody should probably do a proper coccinelle script for this, but for me the trivial script actually resulted in an assertion failure in the middle of the script. I probably had made it a bit _too_ trivial. So after fighting that far a while I decided to just do some of the syntactically simpler cases with variations of the previous 'sed' scripts. The more syntactically complex multi-line cases would mostly really want whitespace cleanup anyway. Signed-off-by: Linus Torvalds <torvalds@linux-foundation.org>
2026-02-21Convert 'alloc_obj' family to use the new default GFP_KERNEL argumentLinus Torvalds3-8/+8
This was done entirely with mindless brute force, using git grep -l '\<k[vmz]*alloc_objs*(.*, GFP_KERNEL)' | xargs sed -i 's/\(alloc_objs*(.*\), GFP_KERNEL)/\1)/' to convert the new alloc_obj() users that had a simple GFP_KERNEL argument to just drop that argument. Note that due to the extreme simplicity of the scripting, any slightly more complex cases spread over multiple lines would not be triggered: they definitely exist, but this covers the vast bulk of the cases, and the resulting diff is also then easier to check automatically. For the same reason the 'flex' versions will be done as a separate conversion. Signed-off-by: Linus Torvalds <torvalds@linux-foundation.org>
2026-02-21treewide: Replace kmalloc with kmalloc_obj for non-scalar typesKees Cook3-26/+22
This is the result of running the Coccinelle script from scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to avoid scalar types (which need careful case-by-case checking), and instead replace kmalloc-family calls that allocate struct or union object instances: Single allocations: kmalloc(sizeof(TYPE), ...) are replaced with: kmalloc_obj(TYPE, ...) Array allocations: kmalloc_array(COUNT, sizeof(TYPE), ...) are replaced with: kmalloc_objs(TYPE, COUNT, ...) Flex array allocations: kmalloc(struct_size(PTR, FAM, COUNT), ...) are replaced with: kmalloc_flex(*PTR, FAM, COUNT, ...) (where TYPE may also be *VAR) The resulting allocations no longer return "void *", instead returning "TYPE *". Signed-off-by: Kees Cook <kees@kernel.org>

2025-08-25scsi: mpi3mr: Fix premature TM timeouts on virtual drivesChandrakanth Patil2-5/+17
Task Management to virtual drives may timeout prematurely when using a static default timeout. Read Abort and Reset timeouts from Device Page 0 and apply the maximum of the firmware value and the default. This fixes premature TM failures on virtual drives. 2025-08-25scsi: mpi3mr: Fix controller init failure on fault during queue creationChandrakanth Patil1-0/+10
Firmware can enter a transient fault while creating operational queues. The driver fails the load immediately. Add a retry loop that checks controller status and history bit after queue creation. If either indicates a fault, retry init up to a set limit before failing. 2025-08-25scsi: mpi3mr: Fix device loss during enclosure reboot due to zero link speedChandrakanth Patil2-6/+13
During enclosure reboot or expander reset, firmware may report a link speed of 0 in "Device Add" events while the link is still coming up. The driver drops such devices, leaving them missing even after the link recovers. Fix this by treating link speed 0 as 1.5 Gbps during device addition so the device is exposed to the OS. The actual link speed will be updated later when link-up events arrive. 2025-07-14scsi: mpi3mr: Serialize admin queue BAR writes on 32-bit systemsRanjan Kumar3-4/+17
On 32-bit systems, 64-bit BAR writes to admin queue registers are performed as two 32-bit writes. Without locking, this can cause partial writes when accessed concurrently. Updated per-queue spinlocks is used to serialize these writes and prevent race conditions. 2025-07-14scsi: mpi3mr: Fix race between config read submit and interrupt completionRanjan Kumar1-1/+1
The "is_waiting" flag was updated after calling complete(), which could lead to a race where the waiting thread wakes up before the flag is cleared. This may cause a missed wakeup or stale state check. Reorder the operations to update "is_waiting" before signaling completion to ensure consistent state. 2025-06-19scsi: mpi3mr: Correctly handle ATA device errorsDamien Le Moal1-1/+19
With the ATA error model, an NCQ command failure always triggers an abort (termination) of all NCQ commands queued on the device. In such case, the SAT or the host must handle the failed command according to the command sense data and immediately retry all other NCQ commands that were aborted due to the failed NCQ command. For SAS HBAs controlled by the mpi3mr driver, NCQ command aborts are not handled by the HBA SAT and sent back to the host, with an ioc log information equal to 0x31080000 (IOC_LOGINFO_PREFIX_PL with the PL code PL_LOGINFO_CODE_SATA_NCQ_FAIL_ALL_CMDS_AFTR_ERR). The function mpi3mr_process_op_reply_desc() always forces a retry of commands terminated with the status MPI3_IOCSTATUS_SCSI_IOC_TERMINATED using the SCSI result DID_SOFT_ERROR, regardless of the ioc_loginfo for the command. This correctly forces the retry of collateral NCQ abort commands, but with the retry counter for the command being incremented. If a command to an ATA device is subject to too many retries due to other NCQ commands failing (e.g. read commands trying to access unreadable sectors), the collateral NCQ abort commands may be terminated with an error as they run out of retries. This violates the SAT specification and causes hard-to-debug command errors. Solve this issue by modifying the handling of the MPI3_IOCSTATUS_SCSI_IOC_TERMINATED status to check if a command is for an ATA device and if the command ioc_loginfo indicates an NCQ collateral abort. If that is the case, force the command retry using the SCSI result DID_IMM_RETRY to avoid incrementing the command retry count.
