The comedi code came into the kernel back in 2008, but traces its
lifetime to much much earlier. It's been polished and buffed and
there's really nothing preventing it from being part of the "real"
portion of the kernel.
So move it to drivers/comedi/ as it belongs there.
Many thanks to the hundreds of developers who did the work to make this
happen.

This patch fixes multiple occurences of the checkpatch.pl warning:
WARNING: Missing or malformed SPDX-License-Identifier tag in line 1
NB. This is an additional patch and does not overlap the patch sent
yesterday for 8255.h. Should have sent them all together.

Now that the SPDX tag is in all comedi files, that identifies the
license in a specific and legally-defined manner. So the extra GPL text
wording can be removed as it is no longer needed at all.
This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text. And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.
No copyright headers or other non-license-description text was removed.

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.
Update the drivers/staging/comedi files files with the correct SPDX
license identifier based on the license text in the file itself. The
SPDX identifier is a legally binding shorthand, which can be used
instead of the full boiler plate text.
This work is based on a script and data from Thomas Gleixner, Philippe
Ombredanne, and Kate Stewart.

The PLX INTCSR register defines are a bit wordy and many of them
are not used anywhere.
For aesthetic reasons, remove all the *_MASK and *_DISABLED defines
and rename the remaining bit defines so they are not as wordy.
Convert all the bit defines to bit shifts.
