Update the macros to output the compared values at hex for easier debugging
when test asserts fail.
We have to be careful not to re-evaluate expressions as they may have
side-effects. So update the code to take local copies and use these for
both the test and the debug output.
Also remove unused IS_SET() macro.

With __install_special_mapping() moved to vma.c, vma_set_range() can be
made into a static function there and is now completely isolated from the
rest of mm.
While we're here, we can also remove the insert_vm_struct() declaration
from mm.h - the function is implemented in vma.c and already declared in
vma.h, and has no users outside of mm.
Also update the VMA userland tests to reflect this change.
No functional change intended.

Convert the test code to utilise vma_flags_t as opposed to the deprecate
vm_flags_t as much as possible.
As part of this change, add VMA_STICKY_FLAGS and VMA_SPECIAL_FLAGS as
early versions of what these defines will look like in the kernel logic
once this logic is implemented.

Add VMA unit tests to assert that:
* vma_flags_empty()
* vma_flags_diff_pair()
* vma_flags_and_mask()
* vma_flags_and()
All function as expected.
In additional to the added tests, in order to make testing easier, add
vma_flags_same_mask() and vma_flags_same() for testing only. If/when
these are required in kernel code, they can be moved over.
Also add ASSERT_FLAGS_[NOT_]SAME[_MASK](), ASSERT_FLAGS_[NON]EMPTY() test
helpers to make asserting flag state easier and more convenient.

So far the userland VMA tests have been established as a rough expression
of what's been possible.
Adapt it into a more usable form by separating out tests and shared
helper functions.
Since we test functions that are declared statically in mm/vma.c, we make
use of the trick of #include'ing kernel C files directly.
In order for the tests to continue to function, we must therefore also
this way into the tests/ directory.
We try to keep as much shared logic actually modularised into a separate
compilation unit in shared.c, however the merge_existing() and
attach_vma() helpers rely on statically declared mm/vma.c functions so
these must be declared in main.c.
