Simple performance improvements:
- inline empty pmap_deactivate() and pmap_collect().
- inline pmap_phys_address().
- provide a real pmap_kremove() implementation, rather than invoking
pmap_remove() on behalf of pmap_kernel().
- do not check for the MMU hole in pmap_prefer() for SUN4M-only kernels.
