Switch the emitted HTML element from <b> to <code> for the fixed

syntax element macros .Nm, .Fl, .Cm, .Ic, .In, .Fd, .Fn, and .Cd.
Adjust both the internal and external style sheets such that
rendering remains unchanged in typical browsers.
